The information and personal details taken by Tea Tree Speech Pathology are confidential and are not shared with any person, company and / or agency without your written consent. Privacy in our clinic is a high priority and taken very seriously.
We collect and store your information in accordance with the Australian Privacy Act.
No paper records are kept on our premises, all documentation is stored electronically, using a secure cloud-based clinical software program.
Information is only accessed by the clinician.
Documentation is stored for 7 years (or 7 years past the client’s 18th birthday) and all digital files are deleted from the cloud systems / back-ups.
We collect information about you for the primary purpose of providing quality services to you to meet your specific needs. This information is used for:
administrative purposes,
invoicing,
providing quality services,
disclosure of information to the NDIS and other government agencies as necessary,
disclosure of information to other health professionals and services to provide multi-disciplinary care to you.
Tea Tree Speech Pathology may use digital transcription after a session to aid in accurate note taking. Additionally, digital data tracking, summarising, report drafting and note taking may also be used to maximise client outcomes and efficiency. This will always be completed in line with the Australian Privacy Act.

I have read the above information and understand the reason for collection of my personal information, the way it will be used and disclosed, and I agree to those conditions.
I understand that this consent form is valid from the date of signing until I provide written notice that the agreement has finished or changed.
I understand that it is my choice as to what information I provide, and that withholding or falsifying information might act against the best interest of the services I receive.
I am aware that I can request access to my personal information on request, and if necessary, correct any information I believe to be inaccurate.
I understand that if in exceptional circumstances, access is denied for legitimate reasons, that the reasons for this and possible remedies will be made available to me.
I understand Speech Pathologists are classified as mandatory reporters, we are mandated by the Child Protection Act 1999, to report a reasonable suspicion that a child in care has suffered, is suffering or is at an unacceptable risk of suffering significant harm caused by physical or sexual abuse.
